Easy to set up. It even has a multiple choice option where the customer can select more than one field. We only need one form at the moment, so free is a great price. Not sure why more people aren't using or reviewing this app. The previous reviewer that gave it a one-star was pretty harsh. I haven't needed support, but when it comes to the Shopify app store, I typically don't expect it for a free...

Awful experience. No customer service / support whatsoever. I started using this for my Shopify site - worked well, but I needed a small tweak, to be able to have a form offer 40 choices rather than the standard 20. Emailed for support -- absolutely no response. It's been weeks. Shopify's customer support even emailed them, & still no response. Now I need to let go of 3-4 hours of work in set...
